I am now listed as an Anthem Provider. I accept most Anthem plans but not Medicare or Medicaid related Anthem plans. If you have any other type of insurance, I will be listed as an out of network provider.
That means I will need you to pay full fee at the time of service. My current hourly rate is $100. I will provide you with a receipt that includes all of the information that an insurance company would need to file an out of network claim. It will be up to you to turn in the paperwork to your insurance company.
Please check with your individual insurance company to see about coverage.
Keep in mind the following:
- HSA plans, even if they are Anthem plans, require upfront payment. I will charge you the Anthem rate for the particular service offered.
- Be aware that you must pay your deductible before Anthem will begin coverage
- Flexible Spending Accounts may be used for out of network coverage.
